Remote Shopping Instructions

Complete the form including your complete mailing address.

Option 1 - Regular Mail

Please attach a check made payable to "SPA Spirit Shop" for the total amount of the order. We do not charge sales tax. Mail the completed form with payment to Summit Country Day School, 2161 Grandin Road, Cincinnati, OH 45208 ATTENTION: SPIRIT SHOP.

Option 2 - Fax to 513-533-5376

Fax the completed form with your credit card information including; account number, full name on the card, and your billing zip code. If you are uncomfortable sending credit card information via fax, then just fax the completed order form and call the Spirit Shop on the next available business day (M,W,F 11:30-1:30) to provide the information over the phone to one of our volunteers. The fax machine is in the Spirit Shop behind the cash register area, and the room is locked whenever the shop is closed.

Please allow up to two weeks for processing and shipment.

Minimum shipping charge is $10 or 10% of total order whichever is greater.

All orders will be shipped via USPS Priority Mail.

Your order will be filled by the Spirit Shop volunteers who work Monday, Wednesday and Friday 11:30-1:30 when school is in session.

Notes: Our box will fit four golf shirts and one sweatshirt or two sweatshirts.
